  • For Prolia and EVENITY, pay as little as $25 out-of-pocket for each dose
  • Can be applied to deductible, co-insurance, and co-payment*
  • No income eligibility requirement

How to use the Amgen SupportPlus Co-Pay Program

Enrollment

  • Enroll online at AmgenSupportPlus.com/copay or over the phone by calling (866) 264-2778
  • Have your insurance information ready when you enroll

Co-pay card

  • If you’re eligible, you can create an account to view card details. You will also receive a welcome letter with a virtual card

Using your co-pay card

  • As advised by your healthcare provider, give the card information to your doctor’s office or specialty pharmacy
  • Your card can be used to help with out-of-pocket costs
