How to use the Amgen SupportPlus Co-Pay Program

Enrollment

  • Create an account online or call (866) 264-2778 to assist patients with enrolling
  • Have patient’s healthcare insurance information ready when assisting patients with completing the enrollment process
  • Patients can enroll with up to 5 products at a time

Co-pay card

  • If the patient is eligible, sign into account to view card details. Patients will also receive a welcome letter with virtual card details

Co-pay card processing

  • Patients give card information to the doctor’s office or specialty pharmacy, as advised by their healthcare provider
  • Healthcare providers can sign into their account to view patient list and status, submit claims, assist new patients with enrollment, and more
